We have to start with what you put into your mouth. Diet plays a pivotal role in managing thyroid health. Gluten and inflammatory grains can trigger autoimmune attacks on the thyroid gland, especially in Hashimoto's. Balancing thyroid, insulin, sex hormones, and nutrients can help the body tap into fat stores for energy, promoting weight loss in individuals with Hashimoto's.

"Losing weight with an underactive thyroid is like running a marathon with a tire strapped to your back," I often say. Addressing underlying issues with insulin, hormones, and diet is crucial alongside fixing the thyroid for effective weight loss.

The Importance of T3 in Thyroid Hormone Replacement

In thyroid hormone replacement, I advocate for mimicking the body's natural processes. Using only T4 in patients who've had a total thyroidectomy is a mistake in my view, or in any thyroid treatment.  T3 is necessary for patients to see improvements in their health. Even a 'baby dose' of 5 micrograms of T3 can significantly benefit patients with no thyroid or those in the end stage of Hashimoto's. Some is better than none!

The Limitations of T4 Monotherapy

Now, let's talk about T4 monotherapy. T4, or levothyroxine, is the standard treatment for hypothyroidism. However, it's not a one-size-fits-all solution. Many patients on T4 monotherapy continue to experience symptoms of hypothyroidism, like fatigue, weight gain, and depression.

Why is this? Well, T4 is a prohormone. It needs to be converted into T3, the active thyroid hormone, to be used by your body. However, not everyone converts T4 to T3 efficiently. Factors like stress, illness, certain medications, and even age can affect this conversion process.

That's why T4 monotherapy doesn't work for everyone. For these patients, adding T3 to their treatment can make a world of difference. It can help alleviate symptoms and improve their quality of life.
